Shouldn't everything protect over santa hat?

I fought a guy with a santa hat in f2p worlds just now, and I killed him, yet I didn't get the hat. How come

Maybe he wasn't skulled....

There are no skulls in the new wildy, and spam forum is full of mean people.

he won't lose his highest valueable item if the activae the "protect item" prayer

 

He will of use the Protect Item Prayer. Although for future reference you can use High Risk Wilderness Worlds where you cannot use that pray (although you have to be a member).

 

They high & low alch for under 100gp, but I'm guessing that protect item is based on G.E value, so that's around 100m. That's why you didn't get the drop.
