Need vouch method help

Hey can someone tell me how to write a proper vouch..

Example i want the vouch to link exactly to the guys post and have that symbol etc that links to it..

 

Ive got 3 vouches helping people deleting recovs from their accounts through MSN...

 

No i mean how to link the vouch to the post and not the thread that's what i'm asking..
