Doesn't look to shabby. There is a lot of space though in your form, might want to make the whole form smaller or add more shit. Good though.
A little word of advice too, upload to a different file host than rapidshare; I, for one, will not download this since it's on RS and it's a pain to dl from there. Here are 2 I use often, and there are unlimited downloads. Oh, and make your link a link next time?
