    The custom/personal ranks are only handed out by admins on special occasions.

    It's hard to answer your question of, "how do you get them?". More or less you hope an admin gives you one.

    Just kiss some admin ass is my recommendation for you.
    Some people get ranks like BMTH doing so.

    Usually by helping/ass kissing amod/admin.

    Or some people are just so well known theyre given them.

    I only know that you have to be friends with the admins.
