Twitch video player banner

Hey guys i'm looking for someone to make me a kick ass Twitch Video Player Banner which is displayed when my channel is Offline. It needs to be 16:9 ratio higher the better resolution wise.
Things i would like to have put on the Page:
my gamertag - xPrincess_Xena
my twitch channel - /xPrincess_Xena
it say Offline somewhere
it asking people to follow so they know when i'm live
maybe a few pictures of game characters from:
Destiny
Call of Duty
Battlefield
division
etc..
don't need a character from every game those are just some games i like to play. you can see my channel here and the current offline image i made sloppy /xprincess_xena
