I've got something to meet the majority of those reqs.

Maxed melee, 91 range, 99 pray, 94 mage, barrows, 93 dungeoneering, 99 wc, 99 craft, 99 fletch, 92 slayer, 83 summon, 83 rc. All other stats 70s-80s with the exception of divination. 287/353 QP.
