You have an idea what a wallet is now, those sites have their own wallets. The money goes to those wallets, and you can transfer the bitcoins from the wallet their to one like blockchain. Coinbase and localbitcoins aren't recommended wallets. They bitcoin exchanges, not wallet providers.

Pretty much impossible to. I've heard of a site that has insane mark up fees. Simply because you are trading bitcoin (irreversible) for paypal (prone to hacks, fraud and chargebacks). Maybe if you buy it from an individual, but you'd need a lot of trust before they accept a reversible payment method for an irreversible one.
