Twitch offline image

Hey guys! i need a graphic done for my new twitch page, i need a banner and a offline image avatar.

Requirments:
my gamertag - Na Skeme
my twitch channel URL - twitch.tv/Na_Skeme
it say Offline somewhere(on the offline avatar)
it asking people to follow so they know when i'm live
maybe a few pictures of game characters from the following games:

League of Legends
World of Warcraft
Counter Strike Source
