Where is the username change request form?

I don't want to be that guy, and trust me I did spend a good few minutes (~15) looking for it, but how would I go about getting a name change. My name... is cringeworthy (was so long ago..)

I know I need at minimum $5 donor to submit the request (I will do this, but want to double check that I qualify for a name change) and to what spectrum I am allowed to change my name.. I don't think there is really a way to fix it.

Is there anyone I can go to/any forum, to make this request?

NOTE: We can in rare cases deny a request without reasoning.
