    • Recovery phone number = (removed)
    • Provide current e-mail address = (removed)
    • Skype/Discord used in the past or present = (removed)
    • If you have donated directly through PayPal = (removed)
      • Transaction date and amount (removed)
      • PayPal email (removed)
    Lost my phone with Authenticator app, have a new one need to readd everyting.
