Account Recovery - Blood SM

I am user 'Blood SM'. I was an established Fire Cape/Fight Kiln service provider many years ago and since then I have lost my necessary login information for said account. Since that point I have:

-moved 3-4 times (so different ip)
-changed my phone number (obviously different number)
-changed email addresses (from similar forgetfulness/lack of book-keeping with passwords)
and so on.

I still have access to my Smokin Mils account and can prove in that way that I am who I say I am, but other than that I am not completely certain how I can prove that I am Blood SM. Any help would be greatly appreciated.

• If your registered birthday is your actual birthday, please provide proof of this in the form of an ID. (Include this post in the background of the picture)
