As the ranked border announcement was made, I was looking around for someone to boost me from D5 60lp to D4 and went with Radeon. He didn't personally complete the boost, it was one of his boosters who, initially after having some trouble, finished it up.

After getting banned, I did some research and found two main script programs, Bot of Legends and joduska. Joduska required an invite to access the forums, so I went on the Bot of Legends forums and found this thread.

As the ban dates coincide with the date of my ban (it's not visible in the screenshot above but if proof is required I'll post it), it seemed like an appropriate place to gather info about when exactly scripts were being used.
Although most of the "when was the last time you used BOL?" questions were answered by "recently", or at least within February, a portion of them also had long-term responses to when BOL was last used. These screenshots are from the first two pages of the thread, all it had from when I posted this report.
The earliest one of these was from June 4th 2015, so to be on the safe side and show that I'm completely not responsible for this, that's what we'll use to see if there's any match history that would show that I had used scripts.
I'm willing to screenshare my match history from that point to a mod, but here's what I caught.

From July 2-12 (2015), I went on a large win streak (something that may happen if I had been using scripts). However, if you look at each and every one of the games, I was duoqueueing with a friend who was on a smurf, and carried pretty much every game as evidenced by damage dealt to champions. To dispel the notion that my friend was the one that was using scripts and thus got me banned, the smurf account he used is unbanned right now. Again, I'm willing to screenshare all of this if necessary, just don't think it would be worth my time to take screenshots of each of the games.

From October 17-20th, again I went on a large win streak (http://i.imgur.com/8tkIKer.png). This was due to the booster playing on my account, as evidenced in the timestamps in my conversation with him matching up to the games played.

Those are the only two instances of me going on large win streaks, which may potentially indicate 3rd party programs. Additionally, from the time of the boost until now I have only played 20 games, none of which were solo/duoq. Because I have come nowhere near using any 3rd party programs on LoL, I am positive that buying the D5->D4 boost was the reason why my account was banned. If any other proof is required please PM me or contact me on skype. As mentioned, I am willing to screenshare anything necessary to prove that the 3rd party programs were not used by me. That's pretty much it, apologies for the wall of text.

 

Send a support ticket to Riot via the support forums and ask them if they can provide you with any more detail in regards to the 'third party programs' that were being used. Don't ask which ones, more so ask what time-frame/games. I doubt they will provide that, but it's worth a shot.

After speaking to Radeon, and other clients who were boosted by the same individual as yourself Jestern, it seems that you were just terribly unlucky in regards to the ban. The booster didn't use any scripts from the people I spoke to, and they also spectated said games.

Another user was boosted from d5-d3, and the booster played around 10+ karthus games, while being spectated, and the user said nothing was script worthy - said user has used varying scripts before. His account remains unbanned.
