How do I fill out Part C on Apples Canadian Tax Form GST506 E?

I emailed this question to Apple support team but they just replied with a generic answer that did not help me. I'm attempting to fill out Apple's Canadian Tax Form GST506 E to allow myself to get paid for my iOS applications however I am a bit stuck with Part C. There is a "Tips for Completing Canadian Tax Form GST506 E" sheet however it doesn't really specify about this section.

Since all of the boxes are blue, does that mean this is something that Apple checks off? Not me? I am not sure if I am the one that should be checking the Ongoing supplies box off. Also do I check off the very first box that starts with "We, the agent and principal, jointly elect for the agent to account for the GST/HST on the supply or supplies described below..."? And what about the section below that that starts with "Describe the nature of the supply or supplies to which this election applies..."? Do I have to write anything in there? The tips page doesn't specify.

 

best bet is to call them. not from canada so idk, i even get confused on use tax forms lol.

 

Meh just fill it out yourself and if they're like "na man we do it" then do it again.

 

Anyone know how long this thing usually takes? Its been like 10 days since I mailed it and I still haven't heard back. And my iTunes connect hasn't changed.
