Account IP locked would like to recover

I was directed here so I can get assistance to recover my account from a past hack/ipban from someone else i got redirected about 3 times from posting on many thread section, to finaly post on in the recovery section. Would like to get my account back finaly, thank you : )

2. Supply us with the current email connected to your account.
3. If you have a recovery phrase, please provide that. (This is something you would have set when you created your account.)
4. Include a virus scan screenshot to ensure that your computer is not infected.
