Hacked account

My account was hacked a little while ago and I reported it from my girlfriends account “Taylurr Wilbon” but now I’m locked out my email is [redacted] please let me know how to unlock it I’m the original owner.

• If your registered birthday is your actual birthday, please provide proof of this in the form of an ID. You may censor out irrelevant information in your picture.
• If your registered email contains your actual name, please do the above as well.
• Include a virus scan screenshot to ensure that your computer is not infected.
