They Wont Let Me Talk?!?!

WELL GUYS I JUST MADE A NEW ACCOUNT AND (sorry for caps lol) i cant use public chat for some parent reason,wtf how can i get to use public chat again????

lol crap i created account for name shop and put random date of birth..lol wow,anyway i can go back and fix this???? it said parent or gurdian click here but it took me to login screen

 

I think that there's a form that you can fill out, and mail to Jagex. Not sure though.

 

any idea where i can find this form???
