Banned for Evasion?

I think I made an account on Sythe a LONG LONG time ago, but I forgot all login details for that account....this is the only account I have access to and I haven't "Ban Evaded" I'm sure this is a misunderstanding....I'd appreciate for this to be taken care of A.S.A.P. as I don't want any people to remember me as being banned and me losing trust...
