E-bullion has this debit card you could sign up for, is it worth getting?
What is the best way to save on the cost of fees to have money in these accounts?

You can withdraw it by selling the egold to company that sells/buys egold like www.icegold.com they can transfer the cash you get from egold to your bank accountits a trusted one i've been using for long time.
