1. My current IP is a Florida IP and is (removed), I have moved around a little bit since I created the account since I am in the military. I originally created the account when I lived in Arizona.
2. The email address associated with the account is (removed)
3. Here is a photo of my Florida Drivers license with my name.

4. I am unaware of any phrases associated with my account or don't remember them.
5. Here is a virus scan I just did 3:56 pm Eastern time here in Florida.

The only thing your ID can prove is that your birthday matches the one on your account, which you have blocked out.

Do you have proof you used to live in AZ?
