Where do you buy it?

 

Go to the Loyalty program and sign in. Then you get the option to purchase them.

 

Judging by the picture, if you're a female character; you can select from a different set of titles. Instead of sir, lord etc.

Edit: And yeah obviously this is from JaGeX's recent update, membership loyalty program.

 

Nope, they're all the same regardless of gender.

Don't buy a name unless you don't bot. Attracts way too much attention.
