Recovering my account

I am currently trying to recover my account but I was wondering what sort of things do you write in this section:

You can write any IP's you have ever logged into the account from, any ISP's you previously had, and significant dates that something happened to the account (ie. bans, mutes). Some old recovery question answers, some old bank pins, any extra passwords you can't fit on the first 3 boxes. basically any information you can remember about your account. if your unsure, post it anyway


EDIT:: membership pins, surfpin pins, payment numbers, uhm... dates that you paid for your membership...

stuff like that

 

Your original recovery questions/answers if you have them. (Only if you got hacked and they changed them)

Your IP like the previous poster said.

 

Put keywords related to your account. Just repeat any information you've entered in other sections such as previous passwords, creation dates, etc. Its like a keyword summary of the entire appeal.

 

tell them wat you have in the bank or wat certain quests are done

 

Could someone please give me a layout which I can follow? I want to make sure I get this account recovered, Thanks!

 

I put ALL of my old trans IDs/PINs I used in there, all of the old IP addresses I used, and all of the times I moved. You can also put the specific date (rather than just the month) of each entry.

 

Last location, name of friend(s) in your list, bank PIN, cash you had, etc.
