How to file a Spam report?

I have heard people talking about how to send one, and I know you click the icon in the top right of the post but what should you say in the report? I looked in a few different places and I couldn't find an answer so I am posting here. Thanks in advance.

 

Type in the offense. Such as if someone is spamming, just put like "spam."

 

Reason why you want to report that post. etc; "spam, flaming, oversized signature" these were only examples.
