When did you create this account? - This step is really problematic. If you don't know when was the account created, try January 2002, if not working, try to recover again - February 2002, etc. Upto December 2003.
TIP: Runescape was created in January 2001. Here's a list of first 2000 accounts created in RuneScape. Press F3 to search for any name. If the name which you are looking for isn't on this list, then you can be sure that the account was created not in 2001.

Additional info: I had only 1 password which I forgot. There were no bank pins, recovery questions and emails registered. I played on this account for a very short period of time (I'm not sure if I've ever logged in).
I also forgot my password. I moved to New York in 2007. I'm not sure if it was June or July.
Now try to recover accounts. If it's locked, then it means that probably many people have already tried to recover it.
