Please provide the following:

• Previous ISP/Log in locations:
• Exact donation date + donation amount and proof of donation (MM/DD/YYYY)
• Recovery phone number: (you have this set)
  
• If your registered birthday is your actual birthday, please provide proof of this in the form of an ID.

I have disabled 2FA on your account.

I strongly recommend re-enabling it as soon as possible, please also write down your backup code in case you lose access to your 2FA app in the future.
