Account was banned ages ago for suspected account hack

i'm not sure how this occurred, but it did several years ago. I have the email associated with the account and I have the original account name. what steps can I take to remove the ban?

 

1. The account was created in [redacted]. I've moved to [redacted] since then & don't have the original IP address. It's been years since the account was compromised
2. the original birthday associated with the account should be [redacted] unless I entered a random date.

3. like I previously stated, the account was compromised years ago, on an entirely different computer.

NOTE: If the email connected to your account includes your name, feel free to expedite the recovery with a blurred version of your ID showing your name. The same goes for if your account has your birthday/year. (We can cross-match this information to make your recovery quicker.)

Unsure of some of the info? Posting from home may be enough, but if it isn't, we can possibly give hints based off of IP

 

Please give me a new and unregistered email address to add onto the account.
