Mother holds knife for protection, and father doesnt like it so takes another knife. The joker (as a kid) sees the whole things and is probably terrified, so looks it. Then his father goes over to him and goes "Why so serious?" (this is where this highly repeated line has its origin) and the father cuts a "smile" onto the jokers face so it looks like he is always smiling. Besides for the immediate lip area, the red on the cheeks are scabs if you look closely from the scars of the permanent smile his father gave him. This is also how the Joker marks his victims.

In another scene he talks about how desperate his mother was financially to pay to fix the Joker's "smile" scars. To show his mother he did not care about them, he shoved a knife or blade down his throat. She got so terrified and scared about her son's action, she left the Joker.

In that other scene, he was talking about him scaring himself, so his wife won't feel so horrible about the scars she had, and her leaving him because of his scars.

Both stories, he made up, so you sir, did not know where he got those scars...
