The whole point of the form is so that we can verify that you know the information, we already have access to all of it. Please re-fill out the form to its fullest.

It would also be helpful to list previous ISPs & locations.

Can you verify the number connected to the account?

 

Number Connected to account would be: (removed), if you wanted the area code.
I'm pretty sure I didn't have a different number at the time.

I've disabled the 2fa on your account. Please re-add 2fa to your account and keep the long code somewhere safe in case you find yourself locked out again. You can use the long code one time as a 2fa code so that you can manually disable it yourself.
