Report A Scammer Section

I know many people do not read the Report a Scammer section, but I do, and I believe that it would be safer for the entire community if we had:

"USERNAME is being reported by REPORTER"

Using this format, it can help those who read that section of this site.

I think a format was planned to be made a few months ago, but was never made. But, things would be much more easier if there was a format that people could fill out to report someone.

 

Maybe something like this?
