mostly id need to know:
recovs (if you know them or are any)
your IP [goood extra infromation]
your ISP ( internet service provider, e.g. comcast, wow ETC.) [good extra information]
when you last logged in
your email
your postal code (E.G. 48309, 48307, those are post codes, something like that anyway)
how many blackmarks it has [good extra information]
If it is stolen, or locked
if stolen, then i need to know the following:
how long its bean stolen [month and year, or day month year is better]
IF ITS MEMBER:
then i need to know your agreement code
and POSSIBLY last reciept for payment, or whatever they ask for... i cant remember what they call it.. oh well dont need it as bad.

FOR UNBANNING / APPEALING
i mostly need to know:
what you did
when
where
possibly why, this forums has a black market, dont worry, i dont care
how you did it
why you think as well you should have it removed, ill combine it with mine to make it information overload 4 jagex ;D
