Twitch Video Player Banner

Hey guys i'm looking for someone to make me a kick ass Twitch Video Player Banner which is displayed when my channel is Offline. It needs to be 16:9 ratio higher the better resolution wise.

Things i would like to have put on the Page:

my gamertag - WackyMackie
my twitch channel URL - twitch.tv/WackyMackie
it say Offline somewhere
it asking people to follow so they know when i'm live
maybe a few pictures of game characters from:

• League of Legends
• Call of Duty
• Maplestory
• Gears of War
• Wildstar

and for the one i like the most i am willing to supply a game on steam i do have Slender The Arrival available to give away to my fav submission.
