Need Form Script

Hello, I have a website which acts like a user submitted archive. I need a script to correspond with forms on a webpage. I need the script to either: Store data in text file(s), or preferably make a new html page from a premade page, with the submitted forms' data in it.

Can anyone help?

you need to look into mysql databases. All the information will be stored in a table in the database then it can be drawn from there.
