Looking to purchase Twitch layout + a few extra pieces for ingame overlays[paypal]

As the title states here is a list of what I need, (included are examples of what I need)

1 x banner 480px high, 1920 wide (only the first 800 pixels in length will be important the rest is just extra bleed off for people with bigger screens)

1 x offline image (1920x1080) Ill scale it for my needs myself

4 or so custom game overlays, I play league of legends, arma 2 dayz mod, trials evolution, and will be playing counterstrike/bf3 and bf4.

I am looking to have a simple elegant design that looks nice
I will ask to receive the PSD's of the project and any accompanying project files.


I am not going to cheap out on this and have a decent budget for this so please post quotes for the work needed below. Also post links to threads/albums of your previous work as well please.
